The problem
Ted has had a cough for the last 2 months. He is finally encouraged
to seek medical advice. The news comes back that his cough was due to tuberculosis.
You want to know how many of your staff has been infected by Ted.
The solution
The Number One Health Group team visits your office and takes blood
samples from all of those who were exposed to TB. All those with a negative
test can be reassured. Those with a positive test require a further assessment
and a chest-X-ray.
If their test is positive, you can rule out active TB by clinical features
and a chest-X-ray. If they have active TB you can initiate treatment.
If they have latent TB you can offer the option of treatment, since there
is a 10% lifetime risk of those with latent TB who have an uncompromised immune
system for developing TB. If they acquire immunodeficiency (ie AIDS, steroids)
or have an existing condition this risk is much higher (i.e. 10% per year
not per life-time).
